As professionals with training and experience in managing individual care needs, our Community Coordinator will provide the information and assistance to plan for the future as well as assist with day to day coordination of services, including:

Accompanying individuals to medical appointments.
Locating housing to fit individual’s financial requirements.
Evaluating and applying for financial programs.
Coordination of services: doctor visits, therapies, meals, home health visits, etc.
Assisting with Reverse Mortgages, long-term care insurance, Medicare.

Community coordination is a collaborative process that includes the client, family caregivers, physicians and specialists, the home health agency, and any community agencies involved. It is the responsibility of the community coordinator to assess, plan, implement, coordinate, monitor and evaluate the options and services required to meet the client’s health needs. The community coordinator is the advocate and communicator for the client in this team approach to total health care.
