Why should I access a physical therapist? Physical Therapists are trained at either a Masters or Doctorate Level and are licensed by the Minnesota Sate Board of Physical Therapy. Our expertise is in human movement dysfunction, and we are also trained in interventions to promote health, wellness, and quality of life. We have a broad knowledge in treating multiple deficits including pain management, balance impairments, postural problems, falls, and strength deficits. Relief of these conditions often results in significant improvements in functional mobility and quality of life. TOP 2. What is the difference between a physical therapist and a personal trainer? While a personal trainer can be an excellent resource for developing and monitoring fitness programs for the healthy population, physical therapists are trained specifically in various disease processes and how they impact human movement related to functional activities. We consult with other health care professionals as necessary in order to ensure all needs of the individual are being met, including issues that affect physical, emotional, mental, and psychosocial well-being. TOP 3. Why pay privately for therapy in my home if I have insurance? It is very common that Medicare and other insurance policies will only cover therapy services when immediate functional progress following an illness or injury is documented. Research shows that even healthy individuals lose strength as they age, which can result in significant functional deficits. The focus of Health Esteem Services is to provide ongoing services to allow our clients to live healthy, active, and meaningful lives in their home and community environments. Even if you were to pay privately for services in a clinic, often times the fees are much higher, the visits may be shorter, and the activities are not performed in your home. TOP 4. Will Medicare be accepted? HES offers services that are typically not covered by Medicare. However, if our initial assessment indicates a problem or condition that is reimbursable through Medicare or other insurance policies, we will make a referral to the appropriate healthcare professional. TOP 5. Why would therapy be continued after Medicare therapy visits? Because of Medicare’s many restrictions on “reimbursable services”, and because Medicare clients must show immediate progress that is reasonable and necessary to improve their level of function, coverage of services is often terminated once a minimum level of independence is achieved. Additionally, Medicare currently does not cover most health, wellness and preventative care services. Research shows that ongoing therapy and participation in a safe and appropriately designed exercise program can improve strength, balance, flexibility, endurance, all which will help reverse normal physical changes that occur as we age. TOP 6. How many times will the physical therapist visit? This is up to you! Once we have completed the initial assessment, we will review the findings with you and make recommendations. Based on our recommendations and your goals, we will determine the appropriate frequency of your visits together. TOP 7. What involvement is necessary from a physician? In the state of Minnesota, Physical Therapists have Direct Access which means that we are able to see clients without a physician’s order. This often results in more efficient, cost-effective care for the client. However, we do believe that in order to receive optimal care, ongoing communication with your physician is essential. We will communicate with your physician following our initial evaluation as well as update him/her on your progress. Finally, because Physical Therapists are well-trained to identify conditions that may need a physician’s attention, we will always refer you to your doctor when appropriate. TOP 8. What kind of diagnoses benefit from physical therapy? Everyone can benefit from some type of physical therapy! People with certain medical conditions such as Diabetes, Osteoporosis, Heart Disease, High Blood Pressure, and progressive neurological disorders such as Multiple Sclerosis and Parkinson’s Disease have specific needs which must be considered when developing the treatment plan. However, research shows that even healthy individuals begin to lose strength as they age, which can result in loss of physical functioning and independence. So, even if you are healthy, a strength training program set up for you by a physical therapist can result in increased independence and can assist with preventing diseases like Diabetes and Osteoporosis. TOP 9. What is the typical client age range? At Health Esteem Services, we do not exclude any individuals who may benefit from our services, regardless of their age. So, the best thing to do is give us a call…we would be happy to discuss your needs, answer your questions, and determine if HES is right for you! TOP 10. What is the common thread for people to access the physical therapy offered by Health Esteem Services? Examples of situations in which an individual may access Physical Therapy through Health Esteem Services include: clients who seek professional guidance in designing a safe, effective exercise program; clients who no longer qualify for Medicare or insurance covered physical therapy, but want to continue to receive services in the comfort of their own homes; clients who have begun to limit themselves from various activities because of fear of falling or fear of injury; and clients who have been diagnosed with cancer we help those who are facing chemotherapy by designing programs which will optimize strength and minimize the fatigue often associated with chemotherapy. TOP 11. Will insurance cover the cost of using Health Esteem Services? Some long term care insurances may cover the cost of physical therapy and nursing services. Each insurance company dictates independently what services will be covered in their policy and how many times the service can be accessed. If your insurance company does cover physical therapy, we can provide the company with the necessary documentation in order to obtain reimbursement for the services. The only way to determine coverage, is to review your policy or contact your insurance company directly. TOP 12. Why receive services in the home rather than a clinic setting? All services are provided in the comfort of the client’s own home ensuring one-on-one attention from the HES staff. By providing services to the client in their own home, the staff is able to include the client’s day-to-day living environment to maximize their therapy. It allows the physical therapist to assess the home environment and assist the client in establishing goals that will maximize their lifestyle choice. TOP 13. What is the cost of the various services? Each service has a set fee and time element specific to that service. Please call for more detailed information. TOP 14. What benefit is a Community Coordinator? The family caregiver benefits in utilizing a Community Coordinator in countless ways. The Community Coordinator assumes the duties as a professional caregiver and reduces the stress for the family caregiver. The Community Coordinator saves time and frustration for the family caregiver by: providing the most current information that will assist with making informed decisions on healthcare, finance, legal, and lifestyle changes; accompanying the client to appointments as well as recording the outcome of the appointments; running errands for the client or with the client; determining which community resources will benefit the client and family. More importantly, this service allows caregivers to spend time with their loved ones rather than focusing on the overwhelming tasks of researching housing, insurance, financing, and medical care options. TOP 16. What can I expect from a Community Coordinator? The Community Coordinator’s responsibility is to provide an individualized service for each client and their family to ease the caregiving burden by guiding the family and client in a manner that will support the client to maximize the their independence in the life style of their choice. A Community Coordinator will guide the family and client through the maze of clinic visits, medical testing, housing options, reverse mortgages, home care, array of community services, legal options, health care directives, caregiving issues, disease education as well as preparation for progression of the disease process. TOP 17. How often is a Community Coordinator used? The amount of time the Community Coordinator is utilized is determined on an individualized basis. Through an assessment, the Community Coordinator will work with the client/family to determine what services will benefit the client to maximize their independence and the needs of the family caregiver. TOP 18. Is Health Esteem Services a licensed service? Health Esteem Services has applied for a Class A Home Care license which allows Health Esteem Services to offer an array of services in order to meet our clients’ needs. TOP 19. What is the geographic area for Health Esteem Services? We provide service in the seven county area surrounding Minneapolis and St. Paul: Anoka, Hennepin, Carver, Scott, Dakota, Ramsey, and Wright. TOP 20. Does Health Esteem Services require a signed contract? A service agreement is reviewed with each client that ensures the client understands what services Health Esteem Services is providing to the client. Health Esteem Service provides ongoing communication with the client which is an essential piece for customer satisfaction. At any time a client may request additional services or alter the end date for services. TOP 21. What is the first step to receive services? Call or e-mail Health Esteem Services and a staff member will follow-up with you immediately to initiate a home assessment at your convenience. TOP 22. If a family member wants to receive communication on the services provided to a loved one, how and when will communication be handled? The client must sign a disclosure form with Health Esteem Services listing the family member who will be the contact person for communication. The staff at Health Esteem Services will contact the stated family member to determine the type of correspondence and the time frame for the communication that the family prefers. TOP |
